[DFDL-WG] Errata doc updated with draft errata 5.39 to 5.59 - from all Trackers

I have responded to your comments with changes and reply comments. I first
accepted all changes, so you can see my refinements easily.

The resulting document has been uploaded as a new revision of the errata
spec for another review cycle:
