[cddlm] Syncing XML schema

As discussed in today's conference, we must synchronize our use of XML
schema and common types.  I would propose that we come up with some
simple names for the schemas and standardize the namespaces.

 

Here is a list of namespaces and schema that are currently used in our
documents.

As Steve mentioned, we also need to add a document that lists our common
faults.
