What do you mean, "burnt into the VM"? 

Lets take OVF for a moment. The macaddress of the adapter is NOT present
there... I would like to be able to specify it to the cloud along with
the OVF to use, or ask the cloud to allocate a unique one to this
instance.

As for how IP is allocated, DHCP is certainly common, but that's a
function of what's inside the VM.  It could also be statically defined.
