I think this has the content in it that I felt I needed to get in;
the proxy section still has some missing text (see mailing list 
discussion) including some of the pro-con discussion.  I also need
help with the authorized proxy responder.

There are many comments left in the document, some of which can be dealt
with as part of a readability edit.
