All current query operation sets have an associated "failed" message.  We  
have redefined the "failed" message as a message having an impact on the  
state machine.  The query message set do not have an impact on the state  
machine, and therefore, should not have a failed message.  These failed  
messages need to be removed and the common error message should be used in  
their place.
