[DFDL-WG] Action 191 - Clarify what constitutes Schema Definition vs. Processing Error

Fixed NaN tbd. Added dynamic/static type error caveats (if unable to
distinguish, use SDE). Added TBDs about decode/encode errors and
dfdl:encodingErrorPolicy.
