This came up as a subtopic of the defaults/optionals/nulls, etc. work.
I have mistakenly been referring to xs:fixed as a facet. it is not, nor
are xs:default, xs:minOccurs, xs:maxOccurs.
Of particular interest is the semantics of the 'fixed' attribute, which is
last in the doc. I picked one of 3 possible meanings, but described the
other two also.
