[cddlm] Faults

I am deep in the wonderful depths of SOAPFaults, as I write my unit tests.

1. The sourceforge repository now has a FaultTemplateClass, wrapping the 
code,qname & error texts for a fault; one of these templates is created 
for every fault in our constants.xml template. This is to make it easier 
to generate full faults, and for test code to verify what they got back.

2. I'm somewhat underwhelmed with Axis2's fault handling right now, but 
it is too late to fix it, so faults will be a bit sparse  at GGF15

3. Flushing out our fault list -informatively- is something we need to 
do. Either it is something we can start discussing at GGF15, or we can 
work on it later.

Before anyone tries to write a client or unit test that interoperate 
with multiple servers, we really need to get our faulting stabilised.
