[cddlm] Survey for OGSA-WG F2F meeting back-to-back GGF15

Steve and the team,

Please fill out the survey below. Hiro is very good in doing these
things, shortly he will post the outcome of the survey and we'll all
know in an obvious way who will attend what and when. I already filled
out the survey. However, FYI I will be leaving on Thursday afternoon. I
plan to attend the OGSA/CDDLM session on Thursday rather than GGF.
